Hello! I can answer questions regarding the New F-1 Change of Information form for students with issued Initial Attendance I-20s and Transfer Pending I-20s from UW-Madison. This form is used for any of the following: - changing your I-20 start date to a future term, - changing your financial information, - changing dependent information on your initial I-20, - reporting that you couldn't get your F-1 visa before the start date on your I-20,  - reporting to ISS that you will not attend UW-Madison.
